There are more than 8,000 medical spas operating in the United States — and the quality varies enormously. A board-certified plastic surgeon running a meticulously supervised practice and a cosmetologist performing filler injections illegally can both call themselves a "med spa." This guide gives you the framework to tell them apart.
01 Start with medical supervision
The single most important credential in any med spa is the supervising physician. All med spas are legally required to have one — but the requirements around their involvement vary by state.
In the best practices, the medical director is actively present, personally performs consultations for new patients, and directly oversees treatments. In lower-quality operations, the medical director may have a business relationship with the practice but rarely or never appears on-site.
Ask directly: 'Is your medical director available on-site today?' If the answer is vague, ask when they were last there.
Gold Standard
Look for a medical director who is board-certified in a relevant specialty: plastic surgery, dermatology, facial plastic surgery, or internal medicine with aesthetic training.
02 Understand the injector credentials
For injectables like Botox and fillers, the person holding the needle matters enormously. Licensed injectors typically hold one of these credentials:
- • MD or DO (physician) — highest level of medical training
- • NP (Nurse Practitioner) — can prescribe and inject independently in most states
- • PA (Physician Assistant) — highly trained, requires physician oversight
- • RN (Registered Nurse) — can inject under physician delegation in most states
Be cautious of treatments performed by aestheticians or cosmetologists — in most states, they are not licensed to inject, and doing so is illegal. Always ask for the specific credential and license number of the person who will be treating you.
03 Review before/after photos critically
Any reputable med spa will have an extensive portfolio of real patient outcomes. When reviewing photos:
- • Look for patients with a similar skin tone, age, and concern to yours
- • Verify that photos are consistent lighting (not just dramatically lit 'after' shots)
- • Be suspicious if the only photos available are from the device manufacturer — ask for their own patient photos
- • Volume matters: a practice with 12 before/afters vs. one with 200 tells you a lot about their experience level
04 The consultation tells you everything
A great consultation is thorough, unhurried, and educational. A red-flag consultation involves a lot of selling and very little listening. What the best providers do:
- • Ask detailed questions about your health history before recommending anything
- • Explain what they can and cannot realistically achieve
- • Tell you what you don't need as readily as what you do
- • Provide a written treatment plan with itemized pricing
- • Never pressure you to book on the spot
Red Flag
If a provider recommends a full treatment package in your first 10 minutes without a detailed assessment, without reviewing your health history, or with a "today-only" discount — walk out.
05 Check for accreditation and licensing
Beyond the medical director and injector credentials, look for:
- • State medical spa license (requirements vary by state but most require one)
- • AAAASF or JACHO accreditation for facilities performing in-office procedures
- • Valid malpractice insurance (you can ask)
- • Active DEA license if the practice uses controlled substances
